An apparently difficult diagnosis of infective endocarditis

Abstract
The paper describes the case of a 7-year-old child with a ventricular septal defect and a story of 10 weeks of non-periodic recurrent fever repeatedly treated with short course of antimicrobial therapy. All the echocardiographies were negative for valve vegetation, but all the cultural blood exams were positive for Streptococcus sanguinis. Infective endocarditis was diagnosed using the modified Duke’s criteria and treated with a long course of adequate antimicrobial parenteral therapy.

L'articolo descrive il caso di un bambino di 7 anni con un difetto del setto ventricolare e una storia di 10 settimane di febbre ricorrente non periodica, ripetutamente trattata con un breve ciclo di terapia antimicrobica. Tutte le ecocardiografie erano negative per la vegetazione valvolare, ma tutti gli esami colturali del sangue erano positivi per Streptococcus sanguinis. L'endocardite infettiva è stata diagnosticata utilizzando i criteri di Duke modificati e trattata con un lungo ciclo di adeguata terapia parenterale antimicrobica.
