Abstract
The objective of our study was to describe the spectrum of clinical features necessary for diagnosis and actual treatment of community-acquired pneumonia in children in northern Italy. It was a prospective observational study of paediatric pneumonia diagnosed in outpatient settings and in 9 hospitals in northern Italy from January to December 2004 in comparison to a previous study made 20 years before in the same area. Our study demonstrated a high rate of clinical diagnosis of CAP made by primary care paediatricians (93%) according to recent evidence-based guidelines on diagnosis and treatment of paediatric pneumonia; a decrease in hospital admissions (1/3 of total diagnosis) for CAP and a lower rate of antibiotic therapy by intravenous administration in hospital admissions (only in the 46% vs 74% 20 years ago). Despite these recent improvements in management of CAP, clinical assessment of children influenced independently management of pneumonia including admission to hospital and route of administration.
