Abstract
Guidelines for the prevention of bacterial endocarditis have been recently revised by
American Heart Association and European Society of Cardiology. By now, in contrast to
previous indications, prophylaxis is recommended only for high risk patients, and only in
case of really invasive dentistry care or procedures of other districts with pre-existing infections.
In this way a lot of patients have suddenly lost their indication for the prophylaxis.
Of course, this fundamental change has already created a great debate in the scientific
community, as it radically changes habits deeply embedded in the daily life of both physicians
and patients. Surely stronger evidence is still needed in order to demonstrate the
efficacy (or inefficacy) of antibiotic prophylaxis of bacterial endocarditis.
