Abstract
Children with medical complexity (CMC) are at increased risk for complications, medical errors, and poor outcomes, particularly during emergencies when time to intervene is limited and critical medical information is difficult to obtain. Emergency information forms (EIFs) have been proposed to provide abstract critical information for optimal care of CMC during emergencies. EIFs are standard forms recommended by the American Academy of Pediatrics that provide pre-planned rapid response instructions, including medical conditions, medications, and care recommendations, for emergency providers.
The paper describes the implementation and use of EIFs in a project called “Emergenza Bambini Fragili” operating in the province of Como (Italy) and aims to the diffusion of EIF utilization in Italy to improve a personalized management and care of CMC during emergencies.
Riassunto
I bambini medicalmente complessi (CMC) sono a maggior rischio di complicanze, errori medici e prognosi infausta, in particolare durante le situazioni di emergenza, quando č necessaria prontezza di intervento ed č difficile reperire rapidamente i dati anamnestici critici fondamentali. La lettera d’emergenza fornisce le informazioni cliniche necessarie per l’assistenza dei CMC durante le emergenze. Essa č costituita da un modello standard raccomandato dall’American Academy of Pediatrics che fornisce indicazioni per una rapida risposta in caso di urgenza, un conciso riassunto clinico-anamnestico, le condizioni cliniche di base, la terapia domiciliare e le raccomandazioni di cura necessarie per gli operatori dell’emergenza/urgenza. Abbiamo descritto la nostra esperienza nell’implementazione e nell’utilizzo della lettera di emergenza nell’ambito del progetto “Emergenza Bambini Fragili” sul territorio della provincia di Como. Ci auguriamo che l’utilizzo e la compilazione della lettera di emergenza possano diffondersi in tutto il territorio italiano allo scopo di migliorare la presa in carico e la gestione personalizzata dei bambini medicalmente complessi durante le situazioni di urgenza/emergenza.
